download the form, print it, fill it out by hand, and send it; you are advised not to take this option if you can use Acrobat;

1. If your contact information has not changed, you need not fill in the contact data again
2. If you are registering as a student member, enclose a proof of your status (e.g. copy of student ID)
3. If you want to pay by bank transfer, pay in advance and enclose a proof of your payment to your registration form
4. If you want to pay by credit card, send the form by fax or surface mail, but not by e-mail!
